The Office of the Registrar verifies a student’s enrollment status and/or academic standing for Club Sports organizations to determine eligibility for participation.

Overview of the Process

  1. The designated club representative or team captain submits the required eligibility verification form, instructions, and eligibility requirements to the Office of the Registrar.
  2. The club representative must obtain written consent from each student participant authorizing the release of information. The consent must include the participant’s handwritten signature and the current date.
  3. All documents must be submitted together to the Office of the Registrar, Transcript and Certification Department at [email protected].

Processing Time: Five (5) business days from receipt of a complete request. We are unable to accommodate short-notice requests, so please plan accordingly.

Step-by-Step Guide for Submission

Step 1 - Complete Your Section of the Verification Form

The designated club representative or team captain must complete their portion of the eligibility verification form.

Information to be included in the request:

  • Name of the club, league, tournament, or sport
  • Eligibility requirements and instructions (may be listed on the form or included as a separate document)
  • Deadline date
  • First and last name of each student participant
  • U-M student ID number for each student
  • U-M semester(s) to be covered in the verification (e.g., Fall 2026)

Important:

  • Leave all sections intended for the Office of the Registrar blank.
  • Submit forms in PDF or Excel format only.

Step 2 - Obtain Written Consent from Each Student

Consent Requirements:

  • Each student listed on the roster must sign and date the submitted verification form or a separate sheet authorizing the release of their FERPA-protected information.
  • Signatures must be handwritten. Typed names, script-style fonts, or copied-and-pasted signatures will not be accepted.

Examples of FERPA Protected Information:

  • Student ID numbers
  • Date of birth
  • Credit hours
  • Grade point average (GPA)
  • Academic standing 

If the verification form does not include a consent section for the student’s signature and date, create and attach a supplemental FERPA release form that includes the authorization statement below, positioned either above or below the student signature lines.

FERPA Authorization Statement:

By signing this form, I grant permission for the University of Michigan, Office of the Registrar, to release my FERPA-protected student information for the purpose of verifying my eligibility and academic standing for [Organization/Event] to [Authorized Recipient], [Title/Role], at [Email Address] or for pickup by [Full Name] at Wolverine Services, 2200 Student Activities Building, 515 E. Jefferson St.

Note: Students must be informed who will receive their FERPA-protected information, and the recipient’s name and delivery method must be included before the form is signed. This information cannot be added after the student signs the form.

Step 3 - Submit Materials

Submit all forms at least five (5) business days before the verification is needed.
